Purpose:To support projects in the following priority areas: Healthy Lifestyles, Oral Health, Preventive Health and Primary Care. Grants are awarded in the following categories: Project Planning; Direct Services, Organizational Capacity Building and System Development/ Policy/ Environment. Healthy Lifestyles priorities are physical activity and nutrition in the following settings: Child Care Centers; Schools; After-schools; Worksite; Senior Centers; and Senior Housing. Areas of interest in the Primary Care category include: Chronic Care Management; Care Integration and Coordination; Information Management; Adherence to Clinical Standards; and Self-Care for Diabetic Patients. Priority topics within Preventive Health include: Cancer Screening (Breast, Cervical, Colorectal); and Hypertension/Cholesterol. Oral Health priorities include: Education; Prevention; Clinical Services; and Public Policy Advocacy.
Preliminary Review Comments:
Applicants must complete the preliminary online application to determine eligibility. This site will be available after July 26. Applicants will be notified by August 27th if invited to submit a full proposal.
